ARCHERY SA Awards Nomination

The first part of the form seeks YOUR details as the NOMINATOR. Questions seeking details about the Nominee will follow. Please ensure you have all the relevant information to hand when completing this form. You cannot leave the form and pick up where you left off at a later time. The completed Nomination will be forwarded to the ARCHERY SA Board Secretary AND the ARCHERY SA Awards Committee. The Committee will consider the Nomination and make a recommendation to the Board of ARCHERY SA. A recommendation for the granting of an Award will include a suggestion as to the type and level of Award to be provided. The decision of the Board will be final. The Nomination will remain confidential, until such time as the Board decides, if an Award is granted.
